Role Description KiActiv® Mentors provide one-to-one support and guidance to people who are participating in the KiActiv® Health programme. This is a paid role which involves supporting people to get the most from their personalised KiActiv® Online Dashboard and its functionality, improving their understanding of their everyday physical activity, and equipping them with the knowledge, skills and confidence to self-care effectively. This role is specific to KiActiv®'s acute care pathways, with a primary focus on cardiac rehabilitation for people recovering from major cardiac events/procedures, and/or living with conditions such as Heart Failure. As such, we are seeking applicants who have a background and qualifications in either exercise physiology or nursing. Applicants will have an interest in health improvement among adults who have demonstrated their readiness to change behaviour and be interested in supporting people to find ways to achieve their goals in the context of their health, capacity and environment using the KiActiv® Health system. A strong candidate for the role as a KiActiv® Mentor will demonstrate excellent communication skills, empathy, objectivity, and the ability to motivate. Successful applicants will be provided with the relevant KiActiv® Mentor Training, a course designed to equip attendees with the knowledge and skills required to deliver KiActiv® programmes. On completion of training, attendees will be able to support individuals across a variety of KiActiv® Health programmes.
Main Responsibilities
- Part-time: 20 hours per week (preferably, this would be split into 5x4 hour working days; however, there is some flexibility here, with weekend and/or evening working hours (if suitable), as well as distribution of working hours throughout the day)
- Deliver 1-to-1 Mentor sessions across the KiActiv® Health programme to clients (primarily via the telephone, occasionally via video call where requested)
- Schedule sessions with clients, keep records of attendance and maintain contact with those who do not attend some sessions
- Efficiently manage their own caseload to ensure clients receive appropriate levels of support and expectations are met
- Motivate, engage, support, and reassure clients in relation to physical activity, whilst being considerate of and understanding barriers to change
- Keep detailed, in-situ notes for each Mentor session in line with service delivery requirements
- Report regularly to your Mentor manager on participant issues, concerns, performance
