Job Purpose:
The post holder will provide one-to-one health and wellbeing support to individuals living with musculoskeletal (MSK) conditions and/or long-term pain. This support will be tailored for those who have recently begun sick leave or are at risk of needing to take sick leave, supporting them to manage their condition and maintain their ability to work.
You will take a person-centred approach, working closely with individuals across Bradford and Craven to build their confidence, provide guidance on achievable lifestyle changes, and connect them with a range of suitable activities and workshops. This tailored support will promote improved health and wellbeing while helping individuals to continue in employment.
Key Duties of the Post:
- To hold a caseload of clients and provide one to one coaching, via phone call, email and face to face to individuals who are currently on sick leave, or at risk of needing to take sick leave.
- To use motivational and behavioural change techniques and coaching tools to support people to make changes that meet their needs and circumstances.
- To communicate with both employers and employees to provide support both in and out of the workplace, offering guidance on reasonable adjustments.
- Advocating between employees and employers where appropriate.
- To support individuals with setting achievable goals that lead to improved health, wellbeing, and persistent pain self-management, alongside job retention.
- Building relationships with employers and health care professionals to encourage referrals and promote the service.
- To liaise with a range of services, internally and externally to identify appropriate support packages and referral routes.
- To work closely with the other internal programmes and health coaches to identify areas of support to benefit the individual.
- To support development of resources for the programme, and to utilise local and national materials to promote ongoing self-care methods.
- To identify community activities and services that enable appropriate signposting for individuals.
- To adhere to the KHL’s policies and procedures including Safeguarding, Data Protection and confidentiality and equal rights policies in all aspects of the work.
- To carry out such other appropriate tasks, as required to meet the aims and objectives of the organisation and the rethinking pain programmes aims and objectives.
Monitoring and Evaluation:
- To collect, collate and maintain data using a range of platforms.
- Provide support to the programme lead in the production of reports for monitoring and evaluation of services.
Special Working Conditions
- Travel around the Bradford District and Craven and when required.

Pay: £26,421.00-£28,770.00 per year
