The Role:
Our service is a busy, supportive environment made up of three integrated teams: Home First, the Integrated Discharge Hub, and the Community Bed Team. We are co-located to provide a strong multidisciplinary approach, ensuring the best possible outcomes for the people we support.
Proposed interview date: 25 and 26 June 2026
Key Responsibilities:
Undertake person-centred wellbeing assessments
Work collaboratively with health professionals and partner agencies
Support timely hospital discharges and reduce delays
Manage a varied caseload within a 7-day service
About You:
Strong communication and interpersonal skills
Able to work at pace and make proportionate decisions
Organised, resilient, and confident managing competing demands
Experience within health, care, or customer-focused environments is desirable
