An opportunity has arisen for an experienced nurse to join the Diabetes Specialist Department, as a Band 6 Diabetes Education Nurse. This will be a job share for 16hrs. This will be based in Gladstone Unit, Wrexham Maelor Hospital.
The post will initially be based in the Diabetes Centre with a focus on delivering specialist nurse led clinics as well as providing expert knowledge and support to community and primary care colleagues, patients and carers. The development of this role is evolving as the approach to delivering diabetes care moves towards an integrated model of care designed to improve quality and efficiency.
He/she will require up to date knowledge and skills in the field of diabetes care and management.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
Diabetes Specialist Nurses (DSNs) are crucial in providing good patient care and promoting self-care management. DSNs work wholly in diabetes care and may be employed in a variety of care settings. A DSN is often the first point of contact for people, referring them to other specialist services.
The successful applicant will be part of a multidisciplinary team that supports change and service development, ensuring individuals with diabetes receive a consistently high standard of care.
Structures are in place for clinical governance, continuous professional development and evidence based practice.
The successful applicant will be expected to deliver evidence based diabetes care working in Chirk.

Person Specification Qualifications
- oRGN Level 1 oNMC current registration oDiploma module in diabetes to Post Grad Level or equivalent oWillingness to undertake further training as required
- Educated to a degree level or willing to work towards
Experience
- oSignificant post registration experience o oExperience working in the field of diabetes oEvidence of teaching or presenting
- oBroad nursing experience across all disciplines within nurses oExperience in research work
Skills and Knowledge
- oExcellent communication and organisation skills oAbility to work independently and to carry own caseload oExtended role of Nurse oGood record keeping skills/documentation skills
